HEALTH WAIVER AND RELEASE ACKNOWLEDGEMENT

The use of At Anchor Day Spa's (the "Spa") facilities and services involves certain risks and hazards, including but not limited to, strains, sprains, allergic reactions, and other injuries or illnesses. Before using the Spa's facilities and services, it is the responsibility of the client (the "Client") to inform the Spa of any health conditions or concerns that may affect the Client's use of the facilities and services.

The Spa is not a medical facility and does not employ licensed medical professionals. While the Spa's staff are trained professionals, they are not authorized to diagnose medical conditions or offer medical advice. Clients with any medical conditions or concerns are required to consult with a licensed medical professional before using the Spa's facilities and services.

The Client certifies that they are in good health and have no medical conditions that would prevent them from using the Spa's facilities and services. By voluntarily using the Spa's facilities and services, the Client assumes all risks and hazards associated with such use.

In consideration of being allowed to use the Spa's facilities and services, the Client, on behalf of themselves, their heirs, and personal representatives, hereby releases, waives, and discharges the Spa, its owners, officers, directors, employees, and agents, from any and all claims, demands, damages, and causes of action arising out of or in connection with the Client's use of the Spa's facilities and services, including but not limited to, any injuries or illnesses that may arise from such use.

The Client acknowledges that they have read this Health Waiver and Release Form, understand its terms, and agree to be bound by its terms. The Client acknowledges that they have had the opportunity to ask questions and clarify any concerns before agreeing to the terms of this Health Waiver and Release Form.

By booking a reservation at At Anchor Day Spa, the Client acknowledges and agrees to the terms of this Health Waiver and Release Form.
